Consultation/Consent Forms.

It is a condition precedent to the right to be indemnified under this insurance that all clients complete, sign and date a consultation form, which includes any necessary written before and after care advice.

This certificate of insurance excludes any claims arising from a client that has not completed, signed and dated the said consultation form.

All other terms clauses and conditions of the policy remain unaltered.
