Acceptance Criteria
  • You the insured or any director, partner or principle are currently a resident of the Republic of Ireland.
  • I agree to maintain client’s records and retain them for at least 5 years. If I have not done this in the past or am just starting in business, I agree to do this now.
  • I have never had any claims made against me.
  • I have not been subject to nor have any pending disciplinary hearings against me.
  • I am not aware of any incidents that have occurred which may have given rise or may give rise in the future to a claim under the policy.
  • I have never had suits for negligence, error or omission made against me.
  • Under current or any previous trading titles, I have never been convicted of any criminal offence, other than motoring, or have any prosecution pending.
  • I have never had any insurer cancel, decline, refuse to renew or only accept on special terms, my liability insurance.

Qualifications*
Please tick the Awarding Bodies for which a qualification is held. We will automatically recognize qualifications from the following Awarding Bodies. If a qualification is from another awarding body, please contact our office for further advice. If the qualification has been gained outside Ireland or UK, an English translation of the qualification certificate will be required. Qualifications must be gained from an attended college course – we do not accept online or distance learning courses. Beauty Therapist must hold a minimum of a level 3 qualification.

Declarations

  • Applicants must provide consultation forms / consent forms for all clients and provide written aftercare advice if necessary.
  • Applicants are suitable qualified to perform the treatments they require cover for, distance learning courses are only CPD not main qualification
  • Applicants will advise of any additional training done within the insurance year
  • Applicants will advise of any changes to staff within the insurance year
  • Applicants will not work with individuals under the age of 16. However, this shall not apply to the provision of complementary therapies or the following list of specified treatments (Waxing excluding advanced or specialized waxing, Spray tan, Manicure, Pedicure and Facials.) to a minor aged under 16 and the above the age of 13 years, subject to the Parent/s or Guardian/s being present and their prior written consent being obtained
  • Applicants if required by the manufacturer of the machine/product must carry out patch tests as per manufacturer’s instructions.
  • Applicants understand it is their responsibility to hold copies of qualification certificates on file for themselves and any employee. In the event of a claim, they will need to have these available for submission if requested
  • Applicants requesting cover for Laser/IPL must have at least 2 years salon experience, hold a CIDESCO qualification or have completed an ITEC Level 4 IPL & Laser course.
  • NVQ level 3 beauticians requesting cover for injectables must hold a skin needling qualification gained via injection or device with a minimum of 6 months practical experience.
